No Maximum Duration:
In theory, there is no set maximum duration for taking THM. -
Recommendations in France (HAS):
THM should be prescribed as long as symptoms persist, with adjustments to the minimum effective dose as needed over time. -
Annual Reassessment:
Each year, the benefit/risk balance should be reassessed, especially after 5 years due to a possible increased risk of breast cancer. -
Therapeutic Window:
It is recommended to have an annual "therapeutic window" — a temporary cessation of treatment once a year to check whether symptoms return. This helps to decide annually whether to continue treatment, particularly after 5 years. -
Extended Treatment:
Many gynecologists extend THM beyond 5 years, considering factors like quality of life, as well as the risks of osteoporosis and atherosclerosis.
